School-based health centers

The School-Based Health Center (SBHC) program in Columbia County provides children and youth with access to primary care and mental health services in order to have healthy students that are ready to be learn.

There are four SBHCs in Columbia County: Clatskanie SBHC, Sacagawea Health Center in St. Helens, Rainier Health Center, and Spencer Health & Wellness in Vernonia. Students can get health screenings, sports physicals, well-checks, immunizations, and mental health counseling among many other health services.

SBHCs are located on school campuses in order to overcome barriers like transportation and to prevent students from missing school as much as possible. We accept most insurances, but no student will be turned away due to their ability to pay.

  • Required disclosure: Patients ages 12-19 who visit the SBHC may be asked to complete an anonymous survey after their visit. The survey asks about the patient's satisfaction and experience at the SBHC, as well as some general questions about their physical and mental health status. Patients can refuse to take the survey and this will not affect their ability to get care at the SBHC. To see a copy of the survey, please go to the "SBHC data Requirements" link at Oregon Health Authority. If you have questions about the survey, please contact the SBHC State Program Office at sbhc.program@state.or.us
