School-Based Health Centers: Learn about our School-Based Health Center Program which specifically offers primary care services to school-aged children and youth.

Dental Care: While we do not have a dental program, we can get your child connected to The Children's Program. The Children's Program is a program that the Oregon Educators Benefit Board (OEBB) and Delta Dental of Oregon (DDOR) created in partnership with Willamette Dental, Kaiser and Oregon dentists to ensure that uninsured children in the State of Oregon have access to dental care. The program will provide basic dental services for children between the ages of 5 and 18. Please ask our staff for more information.

After Hours Access Information:
Most of our offices close after 4:30 but we understand that you might need urgent information after that. We are happy to offer our After-Hours Nurse Advice Line. When you call 503-397-4651 after 4:30, there will be a directory option for the Advice Line. During this call you'll speak with a nurse who will help you for urgent needs.
When Should I Leave a Voicemail Instead of Calling the Nurse?
  • General Information: hours and appointment scheduling
  • Non-Urgent Medical Advice: checking in on labs, etc.
  • Prescription Refills
When Should I Speak With the Nurse?
  • Urgent Medical Advice: fevers, sore throats, injuries, etc.
Are The Nurses Part of The Health Centers?
No. Nurses are part of a contract with CHS, so they will not have access to your records and do not work at any of our sites.
How Will My Provider Know I Called?
Our providers are faxed information about the call. They will follow up with callers based on the nurse's log of the call.
To make sure you are ready for your appointment, please bring the following to your first visit:
  • Photo Identification
  • Insurance Card, if you are insured
  • Completed Registration Packet
