Licensed Family Nurse Practitioner
Join our school-based health centers to serve a population ages 0 years and older! Our clinics provide primary and acute care to all ages and are located in St. Helens, Rainier, Clatskanie and Vernonia. We are seeking a practitioner with a heart for children and youth and some experience with adults as well. Schedules are typically 12 patients or less in a day because CHS providers place additional emphasis on communication and health education in each visit. Many patients have not been seen by a medical provider in an extended period of time so patient care is diverse, complex, and thorough.

Medical Assistant
The Medical Assistant is part of a team that provides care for Columbia Health Services’ health center system. The CHS primary care system consists of two community health centers and two school-based health centers. CHS health centers strive to provide comprehensive, whole-person care by maintaining School-based health center and Patient-Centered Primary Care Home Standards.
 
The ideal candidate will have pediatric experience and experience with electronic health records; CHS uses OCHIN Epic. This candidate will also possess flexibility and effective communication skills.

​Systems of Care Coordinator

This position is responsible for serving as a liaison with the Oregon Health Authority and the local system of care
collaboratives of the North Coast region (Columbia, Clatsop and Tillamook County) This role supports SOC
governance for the regions served and coordinates meetings and communications related to the SOC governance
structure.​

Prevention and Engagement Coordinator
​The role of the Prevention Coordinator is to work collaboratively as part of a small team to engage the community and implement comprehensive evidence-based strategies
Coordinator will be required to network with community partners as a means to strengthen support for the work plan activities. This position requires travel throughout Columbia County as well as in-state travel for meetings and trainings.
