Vernonia School Based Health Center and Clatskanie School Based Health Center. Position(s) will be seasonal, incumbent will work in clinics while school is in session. The Medical Assistant is part of a team that provides care for Columbia Health Services’ health center system. The CHS primary care system consists of two community health centers and two school-based health centers. CHS health centers strive to provide comprehensive, whole-person care by maintaining School-based health center and Patient-Centered Primary Care Home Standards. The ideal candidate will have pediatric experience and experience with electronic health records; CHS uses OCHIN Epic. This candidate will also possess flexibility and effective communication skills. |
Nurse Practitioner/Physician Assistant Columbia Health Services is seeking a Primary Care Provider (Physician-MD/Physician Assistant/Nurse Practitioner) for the clinics it operates in Rainier, St. Helens, Clatskanie and Vernonia. Applicants must feel comfortable practicing in an isolated setting. Columbia Health Services (CHS) operates four clinics in Columbia County, Oregon. The clinics are recognized as Oregon Patient Primary Care Home and certified Oregon School-Based Health Centers. There is fifth clinic at the CHS main office that provides immunizations, family planning, and other reproductive health services. Position is weekdays only. Some vacation coverage and fill-in may be needed at the other clinics. Position requires current Oregon certification and licensure as Nurse Practitioner or Physician Assistant, current BLS certification, DEA. It is the employee’s responsibility to maintain current licensure and certification. Benefits include 12 paid holidays, 2 weeks of vacation (increases every 4 years), in addition to sick leave. Health, vision and dental insurance are available, as is a monthly fitness incentive and a 6% employer match contribution to a 401K. Benefits and leave are prorated based on FTE. COVID-19 Contact Tracer-Temporary Part of Oregon’s framework for Reopening Oregon (Governor Kate Brown’s framework) is to actively suppress the transmission of SARS-CoV-2, the virus that causes COVID-19. To suppress disease transmission in the State of Oregon, it is critical to identify as many people who have been exposed to the virus as possible and then conduct broad Contact Tracing to identify all Contacts that may have been exposed to a case during their infectious period. Community health workers, traditional health workers and individuals who have similar training or skill sets are trusted community members who can effectively build relationships and make Contact Tracing successful. Contact Tracers will utilize community health worker practice and methodology to establish trust with Contacts, assess and collect information required by OHA, and follow scripts, policies and procedures provided by OHA or LPHAs. Contact Tracers will comply with all state and federal HIPAA and other privacy/confidentiality requirements and current investigative guidelines. |
